Volunteer Application

PAW ALLIANCE VOLUNTEER APPLICATION


Thank you for your interest in volunteering with PAW Alliance (Piedmont Animal Welfare Alliance)! We have two programs, A New Leash on Life (NLOL) prison dog-training program and A New Start in Life (NSIL), assisting domestic violence survivors and the pets they love.  We’re honored you’re interested, and we need your help. Our programs’ missions:  “change men’s lives, one dog at a time” and “support domestic violence survivors and the pets they love”  focus our work at the intersection of animal welfare and social justice. This work will change your life, just as it has ours.


A New Leash on Life: PAW Alliance is the animal welfare partner at Forsyth Correctional Center, a minimum-security state men’s prison in Winston-Salem.  A New Leash on Life,  pairs shelter dogs in need of training with qualifying inmate-trainers for 10-week sessions. The trainers are provided weekly education and support by our volunteer instructors. After 10 weeks, the pups are well-trained, loving dogs ready for a forever home. 


Some activities would be excellent for Service Hours for students!


A New Start in Life: PAW Alliance partners with Family Services of Forsyth County by supporting domestic violence survivors through, A New Start in Life. We offer foster homes or secure lodging to pets while their owners are in the local domestic violence shelter. Our largest need for this program is foster homes. Fostering for A New Start in Life is a two month commitment.  PAW Alliance will cover the cost of food and medical care. 


Fosters: We need foster for both programs, A New Leash in Life and A New Start in Life! If you’re interested in being a short-time or long-term foster, you also need to select foster homes under interest and fill out the additional foster home related questions.

Areas of Interest

VOLUNTEER RELEASE AND WAIVER OF LIABILITY


  1.  In consideration for the willingness of Piedmont Animal Welfare Alliance (Hereinafter Paw Alliance) to accept my participation as a volunteer, and for other good and valuable consideration, the receipt and sufficiency of which are acknowledged, I do freely, voluntarily, and without duress execute this Release, and I understand, acknowledge, and agree as follows: 


1. Volunteer Activities. I agree to provide my services to Paw Alliance as a volunteer, and understand that at times my volunteer duties may require my interactions with animals. I further understand that working with animals carries inherent risks including accidents, falls, bites, scratches or allergic reactions.  

2. I also agree that I am not, and will not be considered to be, an employee of Paw Alliance while participating in such Activities. 


3. WAIVER AND RELEASE OF LIABILITY

(a) For myself and for my estate, heirs, legal and personal representatives, executors, successors and assigns, I hereby release, forever discharge, and hold harmless Paw Alliance and its directors, officers, employees, agents, volunteers, successors, assigns, and other representatives (collectively the “Released Parties”), from any and all liability, claims, losses, demands, and causes of action whatsoever that I may have of whatever kind or nature, either in law or in equity, arising from or relating to my participation in the Activities, whether or not resulting from negligence, all of which I hereby waive. Such claims include but are not limited to any claim for any bodily injury, personal injury, illness, death, or property damage arising from or relating to my participation in the Activities, regardless of whether caused in whole or in part by an act or omission of a Released Party. 

(b) I also understand that, except as otherwise agreed to by a Released Party in writing, the Released Parties do not provide any financial assistance of any kind, including but not limited to medical, health, or disability insurance coverage for any volunteer. 

(c) I release, forever discharge, and hold harmless the Released Parties from any claim, demand, or cause of action whatsoever arising from or relating to any first aid or medical treatment rendered in connection with the Activities. I understand and agree that Paw Alliance assumes no responsibility for any injury or damage which might arise from or relate to such authorized emergency medical treatment. 


4. Assumption of the Risk. I understand that the Activities I volunteer for may involve work that may be hazardous to me and inherently dangerous risks, including but not limited to activities such as travel to and from locations where Paw Alliance is performing its services as well as working with animals.  I hereby expressly and specifically assume the risk of damage, injury, harm, or death in connection with such volunteer activities. In addition to any other risks posed by participating or volunteering with Paw Alliance,  understand that working with animals presents some risks, and that there are possibilities of injury that may result from contact with animals, such as scratches, bites, and allergic reactions, which may occur when performing volunteer services. 


5. Transportation. When driving a vehicle to perform volunteer services for PAW Alliance, I agree to maintain at least the State-required minimum amounts of automobile insurance on my vehicle and to indemnify and hold PAW Alliance harmless from any claims arising from and/or related in any way to any PAW Alliance animal’s presence in my vehicle. 


6. Medical Treatment. I release and forever discharge PAW Alliance from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during my time with PAW Alliance. 


7. I further agree that this Release is intended to be as broad and inclusive as permitted by the laws of the State of North Carolina, and this Release shall be governed by and construed in accordance with North Carolina law, without giving effect to its conflict of laws rules.

8. I agree that the sole and exclusive jurisdiction and venue for litigation between myself and PAW Alliance will be a state or federal court having jurisdiction over  Forsyth County, North Carolina. 


9. If any term or provision of this Release shall be held illegal, unenforceable, or in conflict with any law governing this Release, the validity of the remaining portions shall not be affected thereby. 


THIS IS A RELEASE OF LEGAL RIGHTS. READ AND BE CERTAIN YOU UNDERSTAND IT BEFORE SIGNING.

MEDIA RELEASE


I grant and convey to Piedmont Animal Welfare Alliance (PAW Alliance)  all right, title, and interest in any and all photographic images and video or audio recordings made by or for PAW Alliance during my participation in the Activities, including, but not limited to, any royalties, proceeds, or other benefits derived from such photographs and/or recordings. 


I further understand that these images and/or videos may be used on Social Media, in advertising for the organization,  in educational and other promotional materials created on behalf of PAW Alliance. 


I have fully and carefully read this Release, fully understand its contents, and sign it freely and willingly, without coercion or undue influence, and upon full and mature consideration.
